Anna has been engaged in clinical work, university teaching, management and research working in the four UK countries. At a national level she has been involved in Department of Health funded evaluations of professional competence and the role of support workers in the NHS; this has led to further work on professional standards, clinical audit, organisational review, measuring the effectiveness of interventions and patient and public involvement. HPC launches `advert e-kit' for registrants. The HPC has launched a new initiative to help increase public awareness of the organisation and raise the profile of registered health professionals. The e-kit allows registrants to download a special `hpCheck' logo and use it to advertise their registration. The logo can be used on publicity material from websites to stationery. Regulators Launch First Joint Publication The UK's 13 Health and Social Care regulators have today launched a new leaflet entitled, "Who regulates health and social care professionals?" The leaflet is the result of a joint initiative by all of the regulatory bodies including the Health Professions Council HPC ; . The leaflet provides a useful `signposting tool' for the general public outlining which regulatory body is responsible for monitoring each profession and what regulation means. It also provides contact details for all of the organisations. The leaflet will be available online at hpc-uk in large print and in 12 languages. Twenty-five P. falciparum samples were added this year and at present the total number of P. falciparum isolates preserved at the parasite bank is 580. In addition to characterization of P. falciparum isolates for sensitivity to antimalarials, 20 isolates were characterized for size variation in MSP 1 and MSP 2 antigens by nested polymerase chain reaction PCR ; assay. A total of twenty isolates collected from Rajasthan, Uttar Pradesh, Delhi, Orissa, Tamil Nadu were characterized. Maximum number of alleles observed were five in an isolate. Multiplicity of infection in the isolates was about 3.15 by MSP 1 and about 1.95 by MSP 2. A total of nine alleles in MSP 1 ranging between 400 and 650 bp and eight alleles in MSP 2 ranging between 450 and 750 bp were observed. The allelic polymorphism observed in the study samples is shown in Fig. 11. Using these two systems 20 isolates were shown to have 19 different types.

Were the findings demonstrated in systems in which growth was suppressed either by unfavorable temperature or by the use of depleted medium. However, in the former experiment in which it was shown that maximum killing occurred at the optimum growth temperature, another interpretation can be justly made. It is possible that if the antibacterial agent is an enzyme, the temperature of its maximum activity may be nearly identical with the optimum temperature for the growth of the culture. It was necessary, therefore, to test the effect of the saliva on organisms held in a resting state by some other method. Depleted media served this purpose well, wherein it was shown that the salivary factor exerted no apparent killing effect on the organisms. Moreover, when this was controlled by reconstituting the medium by the addition of glucose, the growth of the organisms and the action of the antibacterial agent were similar to the results observed in the complete medium. The results indicate that the salivary lactobacillus bactericidin exerts a lethal effect on sensitive cells only when they are growing. The action of this salivary component upon non-dividing organisms, on the other hand, was without obvious effect. In this respect, the mode of action of this antibacterial factor is similar to other antibacterial agents, such as penicillin, spermine, and polylysine.9-11 Although death due to lactobacillus bactericidin apparently results from unbalanced growth, no evidence is presented suggesting what the basic biochemical impairment may be. Salivary antilactobacillus systems with different physical properties have been reported in the literature. 13 The presence of more than one lactobacillus bactericidin in the saliva is a distinct possibility. Continued studies should be done to clarify this problem. It has been shown that a minimum amount of lactobacillus bactericidin is necessary for lethal action. Under natural conditions, saliva bathes the oral mucosa as a thin film, and it is possible that sufficient concentrations of the factor are not present to have a detrimental effect on susceptible micro-organisms. However, Williams and Powlen, 14 using a roller-tube technique to simulate the in vivo condition, reported that Lactobacillus, as well as other micro-organisms, was unable to survive and multiply in sterile parotid saliva. It has been established that the oral microbial flora is relatively characteristic for an individual.'5' 16 It is postulated that salivary antibacterial systems such as the one described in the present investigation may contribute to the regulation of the oral flora and play an important part in the natural defense mechanisms of the oral cavity. Since this agent probably represents only one of a large number of factors contributing to the antimicrobial defense system of the oral cavity, allied mechanisms must not be overlooked.

MATERIALS AND METHODS Patient characteristics. The study group consisted of 127 patients with AML aged 7 to 55 median 30 ; who were treated from March 1, 1984 through June 30, 1989 at five separate centers with allogeneic bone marrow transplantation BMT ; following preparation with Bu and Cy. Fifty-five individuals underwent transplantation at The Ohio State University Hospitals OSU; Columbus ; , 23 at Wilford Hall at Lackland Air Force Base San Antonio, TX ; , 22 at Hahnemann University Philadelphia, PA ; , 17 at St Vincent's Hospital Sydney, Australia ; , and 10 at the Alfred Hospital Melbourne, Australia ; . This study was designed in 1984 as a single institution OSU ; study and was modified in 1987 to include all institutions known to be using this preparative regimen in all patients with AML. All patients who underwent marrow transplantation for AML using this preparative regimen at the participating institutions are reported. Informed consent was obtained for each patient as required by the Institutional Review Boards of the participating centers. According to the study design, patients were categorized into three groups. Group I consisted of 71 patients with de novo AML in first complete remission. Group I1 included 23 patients with de novo AML who underwent BMT during first relapse or second remission. Group I11 included 33 patients who had never achieved complete remission, were in second or subsequent relapse, or who had a history of chemotherapy, radiation exposure, or a myelodysplastic syndrome preceding the development of AML. French-American-British FAB ; classifications were made by referring physicians using standard morphologic criteria with confirmation at the transplantation center. Remission induction and consolidation regimens varied among institutions. Among the 71 Group I patients, 18 required more than one induction course to achieve complete remission and 25 received one or more courses of consolidation. The clinical characteristics of the patients in each group are described in Table 1. Preliminary information for 45 of these 127 patients has been p~blished.'.~ Marrow donors. All donors were siblings of recipients and gave informed consent before marrow procurement. One hundred twenty-four donor-recipient pairs were HLA-A, -B, and -DR identical as determined by standard serotyping. Their cells were mutually nonreactive in mixed lymphocyte cultures. Kruse's conjunctival findings with Bitot's spots was, however, strongly contested by Berliner '42 ; , who suggested that the conjunctival infiltrations seen by Kruse were merely pingueculae, the changes being limited to the subconjunctival layer and not to the epithelium itself. According to Berliner, other raised spots may also be seen in very old individuals, consisting of yellow concretions of lipoid or hyaline material and white chalky deposits resulting from degeneration of cellular products. In a publication of the same year Kodicek and Yudkin '42 ; expressed themselves as being in agreement with Berliner's criticism but suggested that the conjunctival lesion might be a forerunner of the Bitot spots and a sign of vitamin A deficiency. In view of the present uncertainty in the definition of various types of conjunctival spots, and in the absence of conjunctival biopsies, the authors have pre ferred to include the different observed lesions in one group, which in the table is designated as "localized conjunctival thickening." It appears likely that most of the pericorneal spots observed, although rather similar to the description given above, are not true Bitot's spots, as a large number were located medial to the cornea which is claimed rarely to be the case with the spots observed by Bitot. Investigations of the correlation between plasma vitamin A values and the presence of conjunctival thickening as observed by slit lamp examination have been reported by Ander son and Milam '45 ; , who failed to demonstrate any certain relation between the conjunctival changes and the plasma vitamin A level but noted an increased incidence of con junctival abnormalities with advancing age. The results of our studies are reported in table 1.
