After dilation. The refractive surgery literature is replete with studies that highlight the detrimental effects of glare and the increase in postdilation glare secondary to corneal aberrations after refractive surgery.16 Other studies have confirmed that glare reduced VA after dilation.4, 7, 9, 10 Jude et al.7 tested diabetic patients who were driving and found reductions in Snellen VA after dilation that worsened with the addition of glare. They also showed that using sunglasses after dilation with glare actually worsened the Snellen VA in some patients, which confirmed an earlier study showing sunglasses had limited effectiveness with added postdilation glare.4 The incidence of patients with a postdilation Snellen VA worse than 20 40 was 13.3% 14 of 105 patients ; . The incidence increased to 20% 21 of 105 patients ; with the addition of glare, assuming an ETDRS VA score of 70 as being equivalent to 20 40. The calculated relative risks demonstrate that the closer the predilation Snellen VA was to 20 40, the more likely that postdilation Snellen VA would be worse than 20 40 Table 3 ; . This is a rational finding, as patients whose acuity was worse than 20 likely had some corneal, lenticular, vitreal, or retinal pathology that added to the postdilation VA reduction. Postdilation VA was reduced because of increased optical blur, reduced diffraction, reduced accommodation, and or ; increased glare. There were some exceptions to this. The 1 patient with a predilation Snellen VA of 20 and a postdilation Snellen VA of 20 200 was young and had uncorrected hyperopia; thus the reduction in VA occurred as a result of manifest latent hypermetropia after dilation. Two further patients had a predilation Snellen VA of 20 and a postdilation Snellen VA of 20 50, and both had uncorrected refractive error: one was hyperopic, the other was myopic with astigmatism. The importance of proper refractive correction was illustrated in the analysis of the 82 patients who were within 0.50 D of their current prescription. This study was conducted in a tertiary care ophthalmic hospital. We were able to recruit a patient population with a wide variety of acute ophthalmic pathology. We were unable to assess binocular VA since the BAT standardized glare source could only be used monocularly. An interesting study in the future would incorporate pharmacologic constriction of the already dilated pupils in patients who had postdilation VA worse than 20 40 to determine whether there would be a clinically significant reversal showing improvements in VA and CS. It would also have been useful to incorporate the measurement of aberrations to document objectively and correlate the effect of higher order aberrations with the study's findings. Abstract: Apoptosis mediates neutrophil PMN ; phagocytosis and is influenced by cytokines and the Fas Fas ligand pathway. To determine whether apoptosis of cord blood PMN differs from those of adults, cultured PMN were evaluated by morphological analysis, flow cytometry TUNEL assay ; , and DNA gel electrophoresis. In addition, we studied the effect of anti-Fas IgM or cycloheximide on induction of PMN apoptosis. Spontaneous apoptosis 24 h ; was less in cord blood PMN mean SD; 29 9 vs. adults, 56 14%, P F 0.001 ; . The muscles were soaked initially for 1 hr in physiological saline containing 24Na at 380 C. The outward movement of sodium Creese, 1964 ; was found to be faster in denervated muscle, the half-time being 4 1 min after denervation and 5-4 min for controls. For denervated diaphragms the rate constant k was 0-17 min-' + 0-013 standard deviation of the mean from sixteen muscles ; , while for controls k was 0-13 min + 0-016 16; P 0 01; see Finney, 1952 ; . In another series the effect of antimycin on sodium exchange in the rat diaphragm was tested. We were unable to show an effect on the normal rat muscle, although Kernan 1963 ; has observed a decrease in net extrusion of sodium in normal frog muscles. The sodium exchange of the denervated rat diaphragm was, however, markedly affected by antimycin. The halftime of denervated muscles after 2 hr in solution containing 24Na was 4-3 min mean k from eighteen muscles was 0'16 min-' + 0 010 ; , while the sodium exchange in denervated diaphragms treated for 2 hr with antimycin A 10-5 g ml. ; was 64 min k 0.11 min-1 + 0019, mean from eighteen muscles, P 0.01 ; . In this clinical trial the Clinical Global Impressions CGI ; and the global assessments of the investigators and patients underscore the VAS, WOMAC and SF-36 results on efficacy. In all cases a significant superiority po0: 001 each ; of the verum over the placebo was proven: with regard to the improvement of the level of severity and the condition of the disease, with regard to the efficacy index and in the view of both the investigator and the patient. Eight patients of the verum group were symptom-free at the end of the trial, seven no longer required treatment. The Iboga group of terpenoid indole alkaloids takes its name from Tabernanthe iboga Apocynaceae ; , a shrub from the Congo and other parts of equatorial Africa. Extracts from the root bark of this plant have long been used by indigenous people in rituals, to combat fatigue, and as an aphrodisiac. The root bark contains up to 6% indole alkaloids, the principal component of which is ibogaine Figure 6.82 ; . Ibogaine is a CNS stimulant, and is also psychoactive. In large doses, it can cause paralysis and respiratory arrest. Ibogaine is of interest as a potential drug for relieving heroin craving in drug addicts. Those who use the drug experience hallucinations from the ibogaine, but it is claimed they emerge from this state with a significantly reduced opiate craving. A number of deaths resulting from the unsupervised use of ibogaine has led to its being banned in some countries.
