The information contained here and in the Plan Administration section of this Directory, together with your HMO's Certificate of Coverage and applicable riders, addenda and amendments issued by the insurance company, constitute a Summary Plan Description SPD ; as required by ERISA Section 102. A Health Maintenance Organization HMO ; is generally recognized as a costeffective option for frequent users of medical care. Because HMOs utilize a primary care physician PCP ; and Plan members are required to seek care within a network, HMOs are able to manage costs for regular health care consumers. National City offers a number of HMOs. Your access to an HMO depends on the area you live, as determined by your home zip code. A complete listing of HMOs, their address, phone numbers and web sites can be found starting on page 90. When you enroll for HMO coverage, you generally need to select a primary care physician PCP ; for yourself and each of your covered family members. Your PCP treats all basic illnesses and injuries and coordinates all of your care. You are normally required to have a referral from your PCP to use specialists and health care facilities. You can obtain the most current list of network providers for an HMO Plan, free of charge, by: Calling your health care Plan's Member Services Department, Calling HR InfoLine at 888-881-1121, or Logging on to ncHRonline HMOs require little, if any, paperwork and have low out-of-pocket costs. When you use in-network providers, you pay a set dollar amount copayment ; for many services. For other eligible services, the Plan generally pays 90% after you meet the annual deductible. If you enroll in an HMO, you and your dependents must seek care from network providers to receive benefits. The HMO does not pay benefits if you use out-of-network providers except when you need emergency medical care or the HMO approves the out-of-network care in advance.

It is recommended that people co -infected with TB HIV complete their TB therapy prior to beginning ARV treatment unless they have severe HIV disease CD4 50 mm 3, or WHO stage IV disease or the presence of disseminated TB ; . For these patients the risk of dying of HIV disease even when on proper and effective TB treatment is high. For those with CD4 50-200 mm3 they should start ART after the intensive TB treatment phase, which usually lasts for 2 months. In cases where a person needs TB and HIV treatment concurrently, first line treatment options include ZDV 3TC or d4T 3TC plus EFZ. For pregnant women in their first trimester, use SQV r if available. Except for SQV r, PIs are not recommended during TB treatment with Rifampicin due to their interactions with the latter drug. See table 8 and relenza.

Fixed command sequences such as SDRAM initialization, auto refresh, power down or wake-up and transfers of special data structures are easier to implement in a fixed and preprogrammed manner. These tasks are assigned to the sequencer unit of SDC. To keep the amount of memory low and guarantee a defined device shutdown the power down sequences are not a part inside the standard micro program for refresh and initialization. However special power down sequences are loaded into memory when needed. If the SDC currently processes a transfer controlled by the address command generator unit these sequence will be finished normally and then the new loaded routine inside the micro program storage is executed. Volume Control and Mute - can be controlled from the front panel or using RS-232 serial commands. Unique volume levels can be saved for each input and automatically recalled each time an input is selected. Blank - Located adjacent to the input selector buttons, the Blank button allows the video image to be blanked. When Blank is engaged, the output signal goes to black but the sync signals continue, ensuring that the data display device retains sync lock. When the Blank button is pressed again, the previously selected input once again displayed. The Blank feature can also be engaged or disengaged using an RS-232 serial command. Video blank is functional for Inputs 1, 2 and 3. It has no effect on Input 4 which is a passive input. Digital Freeze Frame - provides a high quality still image, useful for applications that require close examination of a specific video frame. Freeze Frame can be enabled disabled using the front panel Freeze button or using an RS-232 serial command. Freeze is functional for Inputs 1, 2 and 3. It has no effect on Input 4 which is a passive input. On Screen Menus provide easy control over video adjustments including hue, color, contrast, brightness, gamma, sharpness, image size, image position, and edge blanking. The on screen menus also make it easy to verify and adjust advanced settings such as output signal resolution refresh rate sync format, RS-232 control options and reset to factory defaults. System Info is a handy menu option that uses the on screen display to show comprehensive information about the input signal and output signal and scaler settings. Image Size, Position and Edge Blanking Controls with individual horizontal and vertical image adjustments make it easy to precisely fit the active video image to the display area and also provide for multiple aspect ratios. The digital Freeze Frame feature can be activated using a front panel button or via RS-232 serial commands. The Blue Screen feature provides a full screen blue image for set up and testing purposes. The blue screen output signal activated via onscreen menu ; is always available, even when the input signal is missing. Mobility, they nd that cultural capital and property provide better security for individu als than organisa tional assets. In these term s, the managers are the least secure. Sim ilarly, in term s of intergenerati onal mobility, the managers are the least cohesive m iddle class, with a high propens ity for their offsprin g to enter the professio nal class. In this context, they argue that the South East acts as an ` escalato r region' for the m iddle classes, especial ly managers and professio nals. Young people enter the South East for higher educatio n, then move on to their rst jobs, before m oving out in mid-caree r. This process provides a national cohesiv eness to the m iddle classes, but a lack of cohesion w ithin the South East due to the high migratory turnover . The web browser interface can be viewed from any desktop or laptop computer, which has access to the network that an AcuAir panel is attached to. To change screens, setpoints, etc., you simply use a mouse and the keyboard to view and change data. All AcuAir screens that are shown will have a several buttons on the right hand side of the screen. Most screens will have three buttons, and one setpoint box. The three buttons are: [Menu] Clicking on this button will cause the main menu to appear on the left side of the screen. Clicking a second time will cause the menu to disappear. [Start Refresh] [Stop Refresh] This button s ; appears immediately below the [Menu] button. This is actually a toggle button, as it changes function from Start Refresh to Stop Refresh each time it is clicked on. When the button shows as Start Refresh, a setpoint box will appear below it, with a time value displayed. The time value determines how often the display screen will refresh, or update its values. This time value is defaulted to 5 seconds, which means that the display will update all values shown every 5 seconds. Depending on the network connection speed, this updating may interfere with your ability to change data setpoints, because the screen will refresh while you are entering the data. To solve this, simply increase the refresh time. The other alternative is to toggle the Refresh Button. By pressing [Stop Refresh], screen refreshing will terminate, and the time setpoint box will reappear. In this state, the particular screen that you are viewing will never update the displayed values. Changing to another screen will show the most recent values on the new screen, and then will not update again. So in the Stop Refresh state, screens will only be updated when new screens are accessed. This feature should not be used when monitoring for data. Try to find an acceptable refresh time for the Start Refresh state. One screen that DOES NOT have these buttons is: Software Maintenance To leave this screen, click on [Exit] button. Note: Using the [Exit] button takes several seconds, and it will force the password level back to zero. There are numerous screens that have additional buttons to those mentioned here. These additional buttons are self-intuitive, and as such, are not discussed here and relenza.
